Overview
The National Core Indicators (NCI) – Aging and Disabilities (AD™) survey helps the Oregon Department of Human Services (ODHS) make things better for older adults and adults with physical disabilities. Vital Research is working with ODHS on this survey. Vital Research's trained interviewers will meet with adults in person, by video call, or by phone call to learn how well the services they receive meet their needs and how the services could be improved.
If you choose to participate, this survey will give you or someone who represents you the opportunity to share opinions, feedback, and personal experiences about the services received from the state.
Key Details About NCI-AD
Here are some things you or someone who represents you should know about the NCI-AD project:
- You may choose to participate or not. Your choice to do the survey or not will not impact the services you receive.
- The interview takes about one hour.
- The interview can be done in-person, over the phone, or as a Zoom video call. You can pick which way you would like to take the survey.
- You were randomly picked to do the survey.
- Your answers and information will be kept private. The state will not know your answers.
- If you need help to do the interview you can choose a person, like a family member or friend, to help you. Paid caregivers are not usually present at interviews as they provide services you will be talking about. Your caregiver may be present and help if you need caregiving during the interview but may not answer questions for you.
